05 Supported Employment

05-03 Eligibility Criteria

In order to be eligible for Supported Employment services an individual must:

1. meet the criteria for most significantly disabled; and

2. meet the eligibility criteria for the basic support VR program as outlined in the Eligibility Section; and

3. because of the severity of their disability, require intensive supported employment services and extended services in order to maintain competitive employment; and

4. have an identified long term funding source for extended services or a reasonable expectation that sources will become available at the time of transition to extended services.
